Family Care Closure: The individuals and families impacted by Family Care’s closure on February 1st 2018 should be receiving a letter from Oregon Health Authority about your new insurance plan.
Broadway Medical Clinic is committed to continuing the healthcare relationship we have with our established Family Care patients. We understand the health plans being offered are limited to CareOregon, Providence, Tuality Healthcare and Kaiser Permanente. We are contracted with the following two plans being offered; Care Oregon and Providence Health Plan. If you are assigned to CareOregon or Providence, but to a physician outside Broadway Medical Clinic AND want to continue your care with Broadway Medical Clinic please call the Oregon Health Plan to request a change. You may be assigned to Kaiser or Tuality Healthcare. Broadway Medical Clinic is not contracted with these two plans.
